recording on pc with a condenser mic

Hello sorry for my english I got a problem you might help me.
I got a SWAMP condenser mic conected to the S-PHANTOM of SAMSON industry and at the output of this a big jack adapter goes to a cable equiped with small jack and that all goes to my lap top and using soundforge 9 i try to record but the finally sound is very very low with lot of noise.On the other hand I already used that mic connecting it to a mic mate that gives the mic the 48volt and goes to a usb input in my laptop and it work great.
So could you help me using my mic in the first way?
